Healthy Youth Survey
SMS Families,
This year, Selah MIddle School was selected to administer the Healthy Youth Survey. The Healthy Youth Survey (HYS) is a statewide, collaborative effort of OSPI, the State Department of Health (DOH), Health Care Authority (HCA), and the Liquor and Cannabis Board (LCB).
Students in 6th and 8th grade will take the survey in Science on Tuesday, October 14th. The survey is voluntary. Students taking the survey can skip questions and stop participating at any time. If you do not want your student to participate in the survey, you can call the school to excuse your student. Students can also inform their teacher that they do not wish to participate in the survey. Students who do not take the survey will remain in Science and participate in an alternative activity, such as studying or reading. There is no penalty for not taking part in the survey. Your student’s grades will not be affected.
Linked below is an informational handout that goes more in-depth on the survey. Please reach out to SMS if you have any questions.
